Pursuant to Article 13(1)(2) of Regulation (EU) 2016/679 of the European Parliament and of the Council of April 27, 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data and repealing Directive 95/46/EC (General Data Protection Regulation), hereinafter referred to as "RODO", I inform you that:

Your personal data will be processed:

  • for the purposes of providing health and treatment services on the basis of Article 9(2)(h) of the RODO in connection with the performance of treatment activities in accordance with Article 3(1) and (2) of the Act of April 15, 2011 on Treatment Activities in conjunction with Article 24 of the Act of November 6, 2008 on Patient Rights and Patient Ombudsman,
  • for the purposes of providing services in the field of aesthetic medicine that do not fall within the scope of medical activities on the basis of Article 9(2)(a) of the RODO;
  • for the purposes of providing social security and administering social security systems and services on the basis of Article 9(2)(h) of the RODO in connection with Article 54 of the Act of June 25, 1999 on Social Security cash benefits in case of illness and maternity;
  • for marketing purposes as well as profiling for marketing purposes on the basis of Article 6(1)(a) of the RODO in connection with Article 10(1) of the Act of July 18, 2002 on the provision of electronic services and Article 172(1) of the Telecommunications Law of July 16, 2004;
  • for the purposes of responding to the question asked by you as part of your inquiries using forms on https://www.stomo.pl on the basis of Article 6(1)(f) RODO and at. 9(2)(a) RODO;

Recipients of your personal data may be prosthodontic and dental technology laboratories, dental laboratories, other entities performing medical activities and cooperating entities.Your personal data processed for health and treatment purposes will be stored for the period of storage of medical records in accordance with generally applicable laws in this regard, including Article 29 of the Act of November 6, 2008. on patient rights and the Patient Ombudsman; personal data processed for purposes other than health and treatment purposes will be stored for a period of no more than 10 years;You have the right to access the content of your data and the right to rectify, delete, limit processing, the right to data portability, the right to object to processing, the right to withdraw consent at any time without affecting the legality of processing performed on the basis of consent before its withdrawal, whereby in the case of data processed under Art. 9(2)(h) RODO, including in particular to data processed as part of medical records and other data processed on the basis of the above-mentioned premise, the right to erasure, restriction of data processing, data portability, object to processing does not apply;You have the right to lodge a complaint to the supervisory authority, i.e. the President of the Office for Personal Data Protection, if you consider that the processing of personal data concerning you violates the provisions of RODO;Your personal data processed:

  • for health and medical purposes is a statutory requirement,
  • for the purpose of providing non-medical aesthetic medicine services is a condition for the conclusion and performance of the contract,
  • for the purposes of providing social security and managing social security systems and services is a statutory requirement.

The provision by you of data processed for health and medical purposes and for the purposes of providing social security and managing social security systems and services is voluntary, however, the consequence of failure to provide personal data will be the Administrator's inability to provide services.Provision by you of data processed for the purposes of providing services in the field of aesthetic medicine not included in the scope of medical activities is voluntary, however, it is necessary for the conclusion and performance of the contract.Provision by you of data for marketing purposes as well as profiling for marketing purposes is completely voluntary and their failure to provide is not associated with any negative consequences for you, and in particular is not a condition for the provision of services by the Administrator.

Your data processed for marketing purposes on the basis of Article 6(1)(a) of the RODO in connection with Article 10(1) of the Act of July 18, 2002 on the provision of electronic services and Article 172(1) of the Act of July 16, 2004 Telecommunications Law will be processed in an automated manner, including in the form of profiling, in order to adjust the offer of services provided to your individual needs and preferences.
